As a Home Health Aide with Gentiva, you will:
*Assist patients with personal care needs (e.g., bathing, mouth care, shampoo and hair care, elimination needs, transferring, dressing and undressing, and ambulation) and household tasks (e.g., making bed, changing linens, laundry, light housekeeping, etc.) essential to the patient's health in accordance with the personal care plan. Maintain a clean and safe environment for patients.
*Plan/prepare nutritious meals and assist in feeding patients as directed.
*Document services provided in an accurate and timely manner in accordance with Company policy and personal care plan. Maintain patient confidentiality at all times.
*Measure and record patient's temperature, pulse, respiration and blood pressure when ordered and/or directed by the RN or therapist, Supervisor or Clinical Manager and personal care plan.
*Report any changes in the patient's condition or incidents to immediate supervisor or the Clinical Manager in a timely manner.
*Assist the self-directing patient in self-administration of medication only as directed by the personal care plan and immediate supervisor or Clinical Manager where permitted by regulation and State Practice Act.
Qualifications
*Training/testing and current certification/registration required by state and/or federal regulations for the provision of services provided
*Ability to read, write and follow verbal and written instructions
*Effective communications skills
*The ability to provide partial or complete living assistance to patients with assistance from other healthcare workers
