RN CASE MANAGER

Job Summary: Pre-Cert patients, Review Admissions and ensure patients meet requirments, Discharge Planning. Must have good commuication skills and an ability to work well with insurance companies and clients such as Nursing Homes.














EDUCATION:


Graduate of an approved registered or licensed practical nurse program.


Currently licensed as RN or LPN.


BCLS certification required
