MEDICAL SOCIAL WORKER (On-Call; Part-Time)

careers at AltaMedResponsible for the supervision and provision of ADHC social services, including assessment, care plan development and advocacy for each participant. Participates in interdisciplinary team meetings and provides consultation to therapists, families, and participants. Requirements: Masters degree in Social Work. Previous supervisor experience with the ability to communicate effectively. Excellent communication (written and oral) skills required. Experience in working with Case Management of the elderly and multi-disciplinary team environments. Must have xperience and knowledge related to the physical, mental and social needs of the elderly. Bilingual (English/Spanish) preferred. Location: Golden Age ADHC.
