Position Summary
The incumbent of this position is responsible for providing money management and mental health supportive services under the social rehab model to a designated population of chronically mentally ill adults. Incumbent coordinates mental health and financial services for a caseload of 50-150 clients or more. The incumbent provides money management, mental health support services, clinical assessments, crisis intervention, and case management. The incumbent maintains accurate client records, including payee records and clinical records.
Reporting Relationships:
The Representative Payee reports to the Program Coordinator.
Minimum Position Requirements:
AA degree preferred or high school diploma with a minimum two (2) years experience
Two years experience in mental health, bookkeeping or social service.
Understanding of El Hogar's mission.Knowledge of basic bookkeeping practices and the Consumer Driven model of care.
Special Requirements:
Hours of Work: Incumbent must be willing to work irregular hours, holidays, and weekends.
Driver's license: Incumbent must possess a current, valid California Driver's License, Class C or higher.
Miscellaneous: Incumbent must be willing to work with persons infected with communicable diseases.
Physical Requirements:
Ability to sit or travel for extended periods of time. Ability to lift and carry charts up and down stairs.
Quantifiable Dimensions:
Maintain caseload of 50-150 or more clients
Respond to client requests for P& I monies
Provide a minimum of 6 hours direct mental health supportive services per day (75%)
Attend weekly supervision with payee supervisor
Complete El Hogar, Inc. training program within a 9 month period
Principal Accountabilities:
Skill Competency/Service Coordination
Effectively performs assessments in accordance with El Hogar and County Policy
Mainatains all records and documentation (progress notes) needed for program compliance and accountability in accordance with Sacramento County and agency standards
Facilitates and maintains coordination with Medical Team, Drug and Alcohol Team, and Social Security Administration to maintain continuity of care for consumers. Coordinates with other El Hogar programs as indicated.
Developes knowledge of community resources and coordinates services with other providers
Familiar with consumer's treatment plan under the social rehabilitation model and able to determine appropriate money management and mental health goals and interventions with consumers as they progress
Ability to cooperate, work, and communicate effectively with coworkers, supervisors, subordinates, and/or outside contacts in a professional manner conducive to a positive work environment
Case Management/Money Management
Ability to write clearly and summarize activities in succinct progress notes and other charting
Provides assistance to consumers and family members to coordinate and facilitate treatment
Participates in program planning, evaluation and development to improve direct and indirect service
Provides linkage to community agencies, criminal justice system, Social Security Administration, other payee services, and promary health care system
Incumbent is knowledgeable of warning signs and symptoms related to mental health challenges. Incumbent supports compliance and advises MD and Clinical Director when a consumer or another is in danger or the organization is at risk
Adheres to standards of legal and ethical practice and seeks consulation as needed
Ability to manage difficult client situations effectively, seeking assistance when needed
Follows payee check cashing protocol through Administrative Services Coordinator
Consumer Relations, Engagement, & Re-Integration
Provides minimum 6 hours per day direct mental health services applying the consumer driven philosophy
Maintains ongoing contact with all individuals on caseload, a minimum of 1 contact every 3 months or more
Provides coaching, supportive counseling, mentoring, feedback, and positive reinforcement for consumers in areas such as independent living skills, grooming and hygiene, public transportation, money management and other daily living skills
Develops Treatment Plan with consumers to identify consumer's personal needs and goals
Mediates and advocates on behalf of consumers to assist them in obtaining and mainataining housing, health care benefits and other entitlements
Keeps daily appointments and promptly returns calls
Manages pro-med mornings and maintains open window times daily
Works effectively with diverse cultural groups
Participates in agency events including client celebrations, staff retreats and agency meetings
Demonstrates timely and thorough follow through on task completion with minimal prompting form supervisors
Time Managemnt and Organization Skills
Practices effective time management and organizational skills to accomplish agency goals and objectives
Demonstrates ability to handle multiple directives at once and prioritize effectively
Completes progress notes daily
Completes annual re-assessments (R&R) on time and keeps all client authorizations up to date
Is self-directed and looks for new ways to eliminate outdated processes that may no longer be effective
Ensures that all work areas are kept clean and neat
Works effectively under pressure with multiple deadlines and priorities
Demonstrates flexibility and adaptability to change
Reporting and Accountability
Completes progress notes and all collateral documentation daily
Prepares reports or special projects as requested
Keeps direct Supervisor apprised of activities including meeting times, special projects and ancillary activities
Adheres to agency communication protocols and "chain of command"

The incumbent must act in the best interest of El Hogar, Inc. at all times and seek consultation from supervisor and/or management when assistance is needed. The incumbent must maintain corporate and consumer confidentiality
WORKING RELATIONSHIPS/CONTACTS: Primary contact within the organization include Service Coordinators, the Clerical Support Staff, Medical Staff, Supervisors, Managers and Mental Health Consumers
CAREER LADDER: Incumbent may be promoted to Service Coordinator with appropriate education, training and experience
